NON-OHIP SERVICES

Uninsured services in Ontario — also known as Non-OHIPs or third-party services — refer to medical or administrative tasks that aren’t covered by the Ontario Health Insurance Plan (OHIP) because they’re considered non-essential or not medically necessary. These services may include doctor’s notes for work or school (sick notes), insurance and legal form completion, cosmetic treatments, travel health consultations, and fees for missed appointments. Since OHIP does not reimburse these costs, they are billed directly to the patient. Below are Non-OHIP services & it’s charges.
Missed Appointments - $50
We allow one missed appointment as a grace. After that, any additional missed appointments will be subjected a $50 fee. Cancellations made with less than 24 hours’ notice will also incur a $50 fee. This fee must be paid before any future appointments can be booked.
Drivers Medical - $200
We offers Driver Medical assessments for patients who need medical clearance for licensing. The service is thorough, efficient, and handled by our family physicians. The fee for a Driver Medical is $200, and appointments are scheduled to ensure you’re seen promptly. Our team is here to make the process smooth and straightforward .
Record & Report Transfers
>>Forwarding your records to a new doctor – $50
>>For notes beyond the first 20 pages, the fee is $1 per page.
>>Insurance Report (APS – Attending Physician Statement) – $325, typically covered by your insurer.
>> Legal medical report – $250 (usually handled by your lawyer)

Private Services
Consultations and Prescriptions
- Private visit – $125 (applies to non‑OHIP consultations such as travel health assessments)
- Tb forms - $50
- Single or double‑sided forms (1–2 pages) – $65
- Forms over two pages – $125 (includes disability, immigration, insurance and similar forms)
Disability
- Disability Tax Credit Form (T2201 – Section B) – $250 (requested by CRA for the disability tax credit)
